Braces Treatment Going Wrong? Warning Signs, Next Steps and Complaint Routes in Hong Kong (2026)

If braces seem to be going wrong, first separate normal soreness from warning signs. Aching after fitting or adjustment usually settles within days. Lasting pain, shifting teeth, receding gums or white marks around brackets mean: contact your treating dentist, collect your records and get a second opinion from a registered orthodontic specialist.
Feeling that something is "off" during or after orthodontic treatment is common, and not every problem means failure. This guide covers what counts as a real problem, how to tell, what to do, who handles refunds and complaints, and retreatment costs. What counts as braces treatment going wrong?
The British Orthodontic Society notes that teeth sit in a complex biological system of gums, jaw bones and facial muscles, so several things can go wrong[1]. Seven groups:

1. Retainers stopped, teeth shifting
The British Orthodontic Society calls relapse one of the most common problems, more likely when treatment is very quick or retainers are not provided appropriately; the jawbone and gums can take at least a year to settle[1]. The American Association of Orthodontists (AAO) explains that bone remodels throughout life and chewing and speaking make teeth drift, so retainers are likely needed long term. Minor shifts may be handled by adjusting or remaking the retainer, larger ones by a short course of braces or aligners, and severe cases by full retreatment[6]. If your retainer no longer fits, do not force it in or give up on it; see a dentist promptly.
2. Teeth not moving as planned
Slower than expected is not automatically a mistake. East Kent Hospitals (NHS) says missed appointments increase treatment time, and a repeatedly broken brace extends treatment and may see it stopped early[2]. If several visits pass with no visible progress, ask for the reason and a revised plan. In a Consumer Council complaint case published in 2024, a consumer paid about HK$23,000 for 18 aligners; the eighth did not fit and he asked for a refund. After the Council stepped in, the trader refunded about HK$8,000 and ended treatment[7].
3. Root resorption
East Kent's leaflet says moving teeth with braces always shortens the roots slightly, usually by a very small amount[2]. It is monitored with X-rays; if you have never had one during treatment, ask whether you need one.
4. Gum recession
The British Orthodontic Society lists gum disease or recession as possible if teeth are pushed out of the jawbone when space is short[1]. If teeth start to look longer or gums keep bleeding, book a review.
5. White spots and decay
NHS 111 Wales says early decay may show up as white marks on the teeth when the brace comes off, and that treatment will not begin without a good standard of oral hygiene, because it can raise the risk of decay and gum problems[3]. East Kent adds that enamel damage can leave a lasting white or brown stain or decay, and that treatment is stopped early if cleaning does not stay at a safe level[2].
6. Bite and jaw joint pain
The NHS lists teeth grinding, joint wear and tear, a blow to the face, stress and an uneven bite as causes of temporomandibular disorder (TMD), with pain around the jaw, ear and temple, clicking and limited opening; it is not usually serious and generally gets better on its own[4]. Braces are not necessarily the cause, but persistent pain or locking needs a dentist's assessment.
7. DIY or remote clear aligners
The Consumer Council warned in 2023 that skipping X-rays may move teeth out of the alveolar bone and cause gum and bone recession, that photo-based monitoring may let bad bites, tooth necrosis or gum problems become serious before detection, and that overseas dentists may not be subject to Hong Kong regulation[5]. Normal discomfort or a warning sign?
The NHS says teeth may be sore for a few days after fitting, eased by soft food and painkillers[8]. The British Orthodontic Society says aching usually settles within a few days, though some people stay tender longer[1].

Found a problem? Five steps
- Do not stop, cut or discard anything yourself. Contact your treating dentist with when it started, which teeth and what you feel; keep photos.
- Get your records. The Dental Council's Code requires records kept for at least six years after the last treatment, and reminds dentists of patients' rights to access them under the Personal Data (Privacy) Ordinance[10]. The Privacy Commissioner says a data access request is usually made on Form OPS003; a copy must be supplied within 40 calendar days, for a fee that is not excessive[11]. Ask for X-rays, scans or models, the plan, consent forms, visit notes and receipts.
- Ask three questions. What is wrong and why? What are the options, including watching and waiting? Any extra charges? Section 5.7 of the Code requires dentists to explain risks and alternatives, including no treatment[10].
- Get a second opinion. As at 31 August 2026, the Specialist Register lists 75 specialists in orthodontics, with practice addresses[12]. Bring your records. See When is a second opinion worth getting?
- Keep evidence. The Consumer Council advises keeping all receipts and treatment records[13].
Refunds and complaints: who handles what?

What does retreatment cost?
Fixes range from a retainer adjustment to a full repeat course[6], so this site does not estimate individual cases. Reference points:
- Fee ranges with sources: Hong Kong dental fees: braces and Clear aligner cost 2026.
- Assessment alone costs money: Hong Kong Sanatorium & Hospital Dental Centre (from 1 Aug 2026) lists orthodontic record and evaluation (conventional or surgical) at HK$5,000–6,000[17]; Union Hospital (from 1 May 2026) lists orthodontic treatment planning at HK$3,000–5,000 and a specialist consultation at HK$1,500–2,500[18]. Clinic fees differ.
- Replacement retainers may cost extra[9][1].

Choosing your next step
On a budget: public and university routes
Public orthodontic care is very limited. Department of Health general public sessions offer pain relief or extraction[19]. Since 6 February 2026, its hospital dental service has been integrated into the public hospital service, serving mainly in-patients, patients with special oral healthcare needs and dental emergencies[20]. The Prince Philip Dental Hospital does not provide public dental services; teaching patients are screened for suitability, and private patients need an outside referral[21]. The HKU Institute for Advanced Dentistry Multi-Specialty Clinic offers orthodontics for misaligned teeth and facial and dental disproportions[22], delivered by supervised resident dentists on a fee-for-service basis[23]; ask about screening. Looking for a private dental centre
Check whether a centre publishes dentists' qualifications and specialist titles, gives itemised written quotes, and explains retainer and review arrangements. As an example, Home Dental's orthodontics page states that treatment is provided at its Causeway Bay and Tsuen Wan centres; that a dentist must be listed under Orthodontics on the Specialist Register to call themselves a specialist; and that interrupted treatment should be reassessed by a specialist in orthodontics as soon as possible rather than abandoned. Complex cases: jaw problems and surgery
Where the jaws are very mismatched in size or asymmetric, braces may be combined with orthognathic surgery. Nottingham University Hospitals (NHS) says the operation is under general anaesthetic with one or two nights in hospital; pre-surgery braces take about 18 months and the whole course two and a half to three years[24]. Private hospital dental centres such as Hong Kong Sanatorium & Hospital and Union Hospital publish price lists[17][18]; the public service serves the groups above[20]. See Braces or jaw surgery?
Prevention: retainers matter most
- Retainers: The NHS says you will need a retainer after the braces come off to stop teeth moving back, and the orthodontist will say how long to wear it[8]. The AAO says a retainer is typically needed for a lifetime, with less wear over time[9]; East Kent says retainers are meant to be worn for the rest of your life[2]. The British Orthodontic Society advises against starting braces unless you are also happy to wear retainers[25].
- Cleaning: Fluoride toothpaste morning, night and after meals, an interdental brush under the brace daily, and less sugar[8].
- Appointments: Keep every review and report breakages promptly[8][2].
- Before starting: Insist on an in-person examination with X-rays[5].
Frequently asked questions
My teeth moved after braces. What now?
If your retainer no longer fits, do not force it; see a dentist. The AAO says minor shifts may need a retainer adjustment or remake, larger ones a short course of braces or aligners[6].
Can I get a refund if braces treatment fails?
Ask the clinic for a written explanation and refund calculation. The Dental Council does not handle refunds[14]; the Consumer Council can conciliate[15], and claims up to HK$75,000 can go to the Small Claims Tribunal[16].
How long should braces hurt?
A few days of soreness is expected; pain lasting more than a few days after fitting or adjustment means contacting your orthodontist[8].
Should treatment stop if gums recede or white spots appear?
Your dentist decides. East Kent says treatment is stopped early if cleaning stays unsafe, to prevent lasting damage[2]. Improve cleaning and book a review.
How do I find an orthodontic specialist for a second opinion?
The Specialist Register lists 75 orthodontic specialists with practice addresses (as at 31 August 2026)[12]. Bring your records and X-rays.
What if the dentist will not give me my X-rays and records?
Make a data access request, usually on Form OPS003; a copy is due within 40 calendar days, for a fee that is not excessive[11].
